I remember a similar program like this back in the 1970's.  It looks like officials with the Duluth Police and the Duluth Fire Departments are resurrecting the Vial of Life program.

Here's how it works.

Interested people obtain one of the vials from participating locations - a list of which follows at the bottom.  Then, users fill out the one page document that asks for pertinent contact information, medical history, prescriptions, etc.  Then - the paper is placed inside the vial, and the vial is mounted just inside of the refrigerator door.  A sticker that alerts resonders to the vial is applied to the front door.

Where can you get your Vial of Life?
